The Lobster Cay is a territory of the United Kingdom located in the western Caribbean Sea to the south of Cuba, right above the Rook Island and both belong to the Cayman Islands. Established as a territory in 1962, the Cay is thousands of miles away from their ruling country, and these days the islands are all homes to several criminal organisations mainly the Sausage's Army and Ortega Cartel.

The Lobster Cay was first connected to crime when it was used used as an offshore financial haven for wealthy people, as the cay was tropical paradise for them. However, both, the Cay and Rook Island became the home of violent crime since the 1980s, when drug cartels from Colombia as well as local Caymanian gangs set up their operations. An Afro-Latin drug dealer known as Sandman claimed that nothing happened on the Lobster Cay without him saying so, and there was corrupted police presence on the islands in the pocket of the Sandman. Colombian cartels like Sausage's Army often clashed with Sandman and his ally Tone Montana. Sausage Army members often parked along the roads leading to villages, and fired upon their enemies or cops on the roads. Lobster Cay, Rook Island and the Cayman Islands were violent places, and civilians often witnessed shootings.
At the seas, the situation was much the same. Ortega Cartel and Sausage men would attack several boats as they headed north to Miami or to other islands, and there were black market operations that smuggled arms and slave trade in the area. The sea was a dangerous place for merchants or gangsters, as they were bound to be threatened by the cartels on their boats or helicopters. On one occasion, a frigate from the US Navy arrived at Devil's Bay to stop the gun running, but it was quickly destroyed by grenade launcher ammunition by Sausage's militia.
