Johnson was born in the New York neighbourhood of Albany, USA, an area with a high criminal rate. He became associated with the Blood Brothers after coming to Miami, becoming an enforcer in the Industrial Zone area. He was always accompanied by bodyguards, always trailing behind him on foot, or in separate cars; Johnny was paranoid of possible hitmen sent to kill him by the Diaz Gang, who were making a move on Downtown. John was never killed by the Diaz men, however, as his bodyguards were always faster on the trigger.

Johnny appears during the big supplier mission in Industrial Zone, Downtown, Miami, or in the Cayman Islands. John will usually be the guy to greet Tony Montana in for the deal. If it's not him it's Nat. John does not appear anywhere else in the game.

- It is unknown which day nor year John arrived in Miami. It was also unknown if he came there by choice or was invited personally somehow and for some reasons.
- John shares a similar character model with Ziggy.
- He is voiced by the famous rapper and former bank robber, Ice Tea, who now plays a slimy cop in L&O: SVU.
