Hotel Medicine was one of the most luxurious and expensive hotels in Mid Town, officially owned by Eileen Medicine. Due to its level of luxury and it's bars, the hotel was chosen by Don Emilio Barzini himself as his own club. In times of crisis it could also house the Barzini underboss and other important guests. During the year 2006, when times were hard, the Barzini Family was forced to open up a sizeable and bigger club in the hotel to earn more money, and the operation was so successful that it was continued, under the direction of Victor Barzini.

Don Barzini's office was located on the top floor near the elevator and the building was defended by twelve heavily armed guards. In order to disguise the nature of the establishment, Barzini passed it off as a legitimately owned and run hotel that even cops on Barzini's payroll could visit. Due to the twelve guards that patrolled it, the Hotel was one of the hardest buildings to storm, but Don Barzini was rarely to be found in his office, and most of his day to day business was run by club manager, Willy the Pimp. However, Pimp was forced and beaten up to give the hotel, brothels, bars, club, and offices to Corleones, as they almost rule the city in 2011.

In 2015, the DeVazzi Family kicked the Corleone Family out of the hotel and out of Mid Town with some help of Barzinis and now the hotel serves under Carmine DeVazzi.
