As with all similar items, the Dildo is not used in the game beyond unlocking a showdown character and journal page.

The Dildo can be purchased from the Tailor in Gold Town after completion of the mission "The Black Semetery". If it's not purchased, it remains available during subsequent visits to Gold Town.
